**Family Solutions from Fairfax Cryobank was initiated in 1998 to offer anonymous donor sperm at a discounted rate. In 2008 we re-evaluated this donor category so as to include more donors. Some of the donors in this group were in and out of the program before we started gathering extensive personal information. Other donors are in this group in order to balance our inventories. Summary and medical profiles are available for all donors in this group, and several of them have additional information available. Donors will rotate into and out of this category as our inventory requirements dictate.

NOTICE REGARDING CANADIAN STATUTORY HOLIDAYS:
Outreach Health Services is closed on all weekends and Canadian Statutory Holidays. There are no shipping or deliveries available on these days. Deliveries to the GTA will be re-scheduled to Tuesday and Thursdays when any statutory holiday lands on a Monday.

Quality Issue Reports must be received within 30 days of shipment to be considered for refund or replacement.
REFUND POLICY
Effective August 10, 2010
- Samples that have been shipped are the property of the purchaser and are non-refundable.
- Units held in reserve at OHS are eligible for a 50% refund within 1 year of purchase.
- Units purchased from the Special Access Listing (DSSAP), Limited Edition Listing or units that become "Second Pregnancies Only" are not eligible for a refund.
